Episode – 1 : A 61 yr old lady had developed burning urination while she was travelling but gradually her burning sensation while passing urine increased. Not only that her frequency of urine increased too alongwith mild pain at her lower abdomen. After 2-3 days she started getting fever with chills with temperature ranging from 101 to 102 Fahrenheit for which she showed to her local doctor there in Kolkata and urine test was done which showed very high pus cells which clearly indicates URINE INFECTION which in medical term is referred as UTI (urinary tract infection). With travelers, urine infection is common especially with females due to extensive use of western toilets. Now some antibiotics were prescribed along with fever controlling medicines and she was treated.
Episode – 2 : After 2 weeks of completion of the treatment, she developed same symptoms again and went to the doctor back and then he advised urine culture this time which showed she is suffering from UTI again with bacteria called E.COLI which was now resistant to some antibiotics. So a course of iv antibiotics were advised as organism was not sensitive to any oral antibiotic and a 10 days course was completed and she was fine again.
Episode – 3 : She was fine for another 2-3 weeks but she started getting the same symptoms again. But now she was lucky that she was in Navi-Mumbai. She visited to urologist in kharghar after searching online for best urologist in navimumbai, rather say best urologist in kharghar. After hearing her story and symptoms, she was examined and there were features which was suggestive of the source of infection which to her surprise was not from outside but from inside her body only. So she was advised ultrasound of abdomen and tests for diabetes. As expected, she had large 18mm right kidney stone and she was borderline diabetic means her diabetes has just started. So in her case, this large stone in kidney was the source of her recurrent infection.
TREATMENT : she got operated for the kidney stone with surgery name MINI-PCNL (a very small or a key hole surgery into the kidney to remove kidney stone) with antibiotic coverage and she was discharged after 2 days as her antibiotic course was going on. MINI-PCNL, the key hole surgery was done in view of infectious stone expectation and yes, we were right, it was indeed an infectious stone (which was also confirmed by stone analysis report which came 8 days after surgery). She could have been operated with different modality or u can say different method called as RIRS (Retro-grade intra-renal surgery) which does not involve making hole into the kidney but it carries a little higher risk of flaring up of infection, multiple sittings to remove such a large stone and yes it is little costlier affair. She got her DJ stent (put during surgery) removed after 2 weeks with absolutely no pain at all.
Follow up: Now after 4 months she followed up. She is absolutely fine with no infection again and her diabetes status has also been reversed.
